Yes and Yes.  We use Jotform, one of the most reputable survey vendors.  We won't share any of your company information with any outside entity.  You'll receive a unique site code for each survey.  Nobody else will receive the survey results of your site code.  All survey results will be anonymous and only shared back with the person who requested the survey at the email address provided.
