Think about a recent problem you faced in your organization.  (Hint: not something too complex)  Walk through the 4 questions you could have asked to drive action, as well as the answers to the questions.  In the real world, was the team able to drive down the issue as far down as possible (ideally to a preventive action)?  Why or why not?
